
EmSculpt NEO is FDA cleared to treat the abdomen, buttocks, thighs, arms, and calves. Those five regions cover most of the areas people want to firm and define, but they do not respond identically. The abdomen tends to produce the most visible change. However, the limbs produce smaller absolute change on a smaller canvas, and the buttocks change in shape rather than in size.
Knowing which is which before you book matters more than most practices let on. Below is what each area can realistically do, and where expectations tend to run ahead of the treatment.
The Five Cleared Treatment Areas
EmSculpt NEO holds FDA 510(k) clearance for non-invasive lipolysis and circumference reduction of the abdomen and thighs, for improvement of abdominal tone and strengthening of the abdominal muscles, for strengthening, toning, and firming of the buttocks, thighs, and calves, and for improvement of muscle tone and firmness in the arms. A separate 2024 clearance covers neuromuscular stimulation in the arms and legs for rehabilitative purposes.
In practice that means the applicator is placed on the abdomen, buttocks, thighs, arms, or calves. Within those regions the applicator position can be adjusted. Treating the arms covers both biceps and triceps and treating the thighs covers inner and outer aspects. It does not mean the device treats any body part you can name.
1. Abdomen
The abdomen is the area EmSculpt NEO was built around and the one where the combination of radiofrequency and electromagnetic energy makes the most sense. There is enough subcutaneous fat for the RF component to work on and the abdominal wall is a large muscle group that responds well to supramaximal contraction.
What people notice is usually a flatter, firmer midsection with more definition through the upper abdomen, and a change in how clothing sits at the waist. What they are describing is two things happening at once: a reduction in the fat layer and an increase in the thickness of the muscle beneath it.
Where the treatment tends to disappoint is when the abdominal fullness is not subcutaneous. Fat that sits deeper, behind the abdominal wall, is not what this applicator reaches. A midsection that projects for that reason will not flatten the way someone hopes. This is one of the more common mismatches we see and it is straightforward to assess in person.

2. Buttocks
Buttock treatment works differently from the abdomen. Therefore, understanding why prevents most of the disappointment around it. The gluteal muscles are being strengthened and built, not slimmed. Nobody comes in asking for a smaller butt, so the goal here is lift and shape rather than reduction.
The result is a firmer, rounder, slightly more elevated contour that comes from the muscle underneath filling out. It is a real change but a subtle one in photographs. It shows up more convincingly in how clothes fit and in profile than head-on.
One thing worth saying plainly: this is not a Brazilian butt lift and it is not a substitute for one. It adds muscle volume. It does not add fat volume or reposition tissue. If you are looking for a substantial change in projection, this is not the treatment that delivers it.
3. Thighs
Thighs are cleared for both the fat-reduction and the muscle-building side of the treatment. This makes them one of the more versatile areas. Outer thighs and inner thighs behave quite differently, though.
The outer thigh usually has a workable fat layer and a large muscle group beneath it, so it behaves somewhat like the abdomen. The inner thigh is thinner-skinned, carries less fat in most people, and is notoriously resistant to change through training. It responds, but the absolute change is smaller and expectations should be set accordingly.
Thighs, arms, and calves all share a set of practical considerations that the core does not. This includes applicator fit to how the session actually feels. We go through those in detail in our guide to EmSculpt NEO for arms, calves, and thighs.
4. Arms
Arm treatment covers biceps and triceps with the applicator repositioned for each. The clearance here is for improvement of muscle tone and firmness rather than for fat reduction, which is a meaningful distinction: the arms are treated primarily as a muscle-building area.
For patients whose concern is soft, undefined upper arms, that is often exactly right because a good part of what reads as softness is a lack of underlying muscle definition. For patients whose concern is loose skin along the back of the arm, it is not. No amount of muscle building will tighten a skin envelope. That is a separate problem with separate answers.
The arms are a smaller canvas than the abdomen, so the change is proportionally similar but visually smaller. People who go in expecting the same order of transformation they saw in an abdominal before-and-after tend to undersell what they actually got.
5. Calves
Calves are the area people are most surprised to learn can be treated. It’s also the one with the most varied motivation behind it. Some patients want more definition in a stubborn area. Others have a leg that never developed proportionally (sometimes after an injury or a long period of reduced activity) and want to even things out.
The calf is a dense, hardworking muscle group with relatively little overlying fat. The treatment is essentially a muscle-building one here. Contractions in the calf feel different from contractions in the abdomen, more concentrated and more like a cramp releasing. It is manageable, but it is worth knowing in advance rather than discovering it in the chair.
Why Some Areas Show More Change Than Others
Two factors explain most of the variation between areas. Neither is about the device:
The first is how much fat is present to work on. The radiofrequency component heats the subcutaneous fat layer; where that layer is thin, as on the calves and inner thighs, there is simply less for it to do. Those areas get most of their benefit from the muscle side of the treatment.
The second is muscle size. A larger muscle group has more room to grow in absolute terms. A proportionally identical increase looks dramatic across the abdominal wall and modest across a bicep because the starting canvas is a different size.
None of this means the smaller areas are not worth treating. It means the abdomen sets an expectation that the limbs will not match visually. Going in aware of that is the difference between being pleased with a result and being underwhelmed by a good one.
What EmSculpt NEO® Cannot Treat
Just as useful as the list of cleared areas is the list of things people ask about that fall outside it:
- The face, neck, and under the chin. Not cleared and not appropriate. Submental fullness and facial laxity have their own treatments.
- The back, chest, and bra line. Not cleared treatment areas for this device.
- Loose or hanging skin. The treatment firms what is beneath the skin. It does not remove or substantially tighten excess skin, and after significant weight loss that distinction matters a great deal.
- Overall weight loss. This is a contouring treatment applied to specific areas. It is not a weight-loss intervention and should not be approached as one.
- Cellulite. Dimpling is caused by fibrous septae tethering the skin, which this device does not address.
If what is bothering you is on that list, say so at consultation! There is usually a better route and we would rather point you to it.
How We Decide Which Areas to Treat
Most people arrive with one area in mind and leave with a plan that either confirms it or adjusts it. The assessment looks at where the fat actually sits, how much muscle is already there, what your goal is in plain terms, and whether the change you are describing is achievable in the area you are describing it in.
Treating more than one area is common. Each area is a separate course of sessions, so a two-area plan is a larger commitment of both time and cost than a one-area plan, and it is worth being deliberate about it rather than treating everything at once because it is available.
